“CHW worked with client who had an eviction and has been living in a hotel for 3 years. Through persistence, CHW helped client get paper work together to be approved for housing. CHW worked with partners at host site ti find resources to help pay off eviction as well as a housing resource list. Client was encouraged to go through the list 20-30 minutes a day. Within a week, client was able to find housing!”
