Since 2002, CHC has improved health in Kansas City by engaging community to identify and address health inequities, amplifying community voices, and serving as a community convener, organizer, and intermediary.
In 2027, CHC will celebrate its 25th anniversary! A lot has changed since 2002, and as we change and grow, so do our mission and vision. Moving forward, these include:
MISSION The Community Health Council of Wyandotte County exists to enhance access and equity to meet the health-related needs of the population we serve.
VISION We envision a thriving Wyandotte County and Kansas City metro where every resident enjoys equitable access to quality healthcare and the opportunity to live their healthiest life, regardless of background or circumstances.
These guiding principles are significant - shepherding CHC’s activities and decisions while aligning with our core values.
Objective: Ensure programs and operations remain responsive to community need, inclusive of community voice, and accountable to those served.
STRATEGIES
Determine geographic service criteria
Launch annual listening campaigns
Establish a Community Advisory Council in addition to Community Action Boards
Implement comprehensive social media strategy
Develop community canvassing programs.
Objective: Optimize Board of Director structure and governance protocols for ideal organizational governance.

Implement full board onboarding

Objective: Increase sustainability by diversifying funding sources.

Secure at least three new institutional funders
Grow individual donations to 5% of budget
Develop standard pricing models for fee-based partnerships
Provide annual fundraising training
Objective: Align programs with Community Health Improvement Plan (CHIP), continue being responsive to community need, and address long-term drivers of inequity.

Align 100% of programs with health priorities
Implement data dashboards tracking outcomes and disparities
Formalize three new partnerships annually
Add 10 new Community Health Workers
These strategic efforts will produce a healthier population with measurable reductions in health disparities across Greater Kansas City, evident in improved health outcomes data, increased access to care, narrowing equity gaps, and stronger care networks. CHC's impact extends beyond service delivery to systemic change: transforming how health equity work is resourced, governed, and held accountable to communities served.
