Oziel Pruneda | Community Health Worker & Health Education Expert

Oziel Pruneda is currently a Community Health Worker (CHW)/Health Education Specialist (HES) at The Community Health Council of Wyandotte County. Oziel has been previously sited at Health Partnership Clinic in Olathe where he primarily served as a general CHW identifying and addressing patient Social Drivers of Health (SDoH) since March of 2024. Oziel focused his efforts on providing patients with an equitable connection to resources in the community.

Oziel officially started his career in population health as a Community Health Worker at KC Care Health Center serving as a health liaison for Saint Luke’s Health System where he co-wrote a healthy lifestyles program aimed at improving health outcomes designed for the members of the Boys and Girls Club Breidenthal Unit in Wyandotte County back in March of 2020. Oziel has had the opportunity of working with major health systems like Saint Luke’s Health System, Children’s Mercy Hospital, Olathe Medical Center, and Federally Qualified Health Centers (FQHCs) like KC Care Health Center and Health Partnership Clinic aimed at increasing health literacy, identifying and addressing SDoH needs through integrated PRAPARE assessments to improve overall health outcomes, medication adherence, and relationship between provider and patient.

Oziel’s efforts outside of the clinic currently lie on advocacy and sustainability of the CHW profession in the Kansas City metropolitan area. Serving his interests, Oziel has become involved with the Advocacy/Sustainability Committee of the Kansas Community Health Worker Coalition (KCHWC). Oziel hopes to represent the vocation by expanding on the needs of today’s CHWs through the CHW Collaborative as Co-Chair of the CHW Forum.
