CHW Success Stories: Guadalupe, September 2016

Through a generous grant via the United Health Foundation and in cooperation with KC Care Clinic, CHC of Wyandotte County’s Community Health Worker initiative seeks to improve health across Wyandotte County. From time to time, we want to showcase some of the success stories from our health workers:

“My client was referred to me by the Nursing staff of Family Planning from the WyCo Health Department on 8/2/16. The client was referred because her high blood pressure at the moment and lack of Primary Care Provider. The client also has some dental issues and vision concerns as well. I referred the client to Duchesne clinic where they are taking care of her, providing a physical exam, and are checking her blood closely to determine if she needs high blood pressure medication. In that case, the clinic can provide for her. Duchesne is also helping with dental care. I referred her to The Health Navigators in El Centro who have helped her to obtain her bifocal eye glasses through the medicine cabinet program. The client has been in the program for 90 days and will be graduated soon.”

Guadalupe Tredway serves Wyandotte County’s English and Spanish speaking communities.
